In eliminating mental health stigma, we encourage standing up to mental health stigma. We are committed to removing systemic barriers, which individuals from marginalized and racialized communities often experience. It is our role to break down these barriers. SMART CLINIC brings a collaborative program between addiction and mental health treatment programs and primary care, which can improve the delivery of services and support to those in need. Our purpose is to provide excellence in treatment to enable all those suffering from addictive disorders to achieve a goal of life-long recovery. To achieve better outcomes for those suffering from addiction and mental health concerns, services from primary to mental health work together to improve the overall wellbeing of individuals. A comprehensive view can put patients on a personal journey to recovery to make healthy changes in their lives, respond to their own needs and establish their own goals. Treatment has a positive outcome when approached holistically with multiple collaborating partners. At SMART Clinic, Collaborative partnership between primary care, substance use disorder, including integrating mental health services within primary care settings. Collaborative models have improved access to mental health care and increased the capacity of primary care to manage mental health and addiction problems. Successful projects in Canada and other countries have demonstrated better clinical outcomes, more efficient use of resources, and an enhanced experience of seeking and receiving care. To put the collaboration in effect, SMART Clinic will be providing experienced health professionals with expertise in addictions and related concerns. Make sure individuals with issues affecting their overall health have access to essential services promptly. Professionals would be working closely together and building trust with individuals who can lead to prudent and more efficient use of resources while supporting the person and their families.
